Consent for Sharing of Student Records

I/We authorize St. Bonaventure's College Admissions to request academic, attendance, and relevant support records from the student's current and previous schools as part of the admissions review process.

Consent for Field Trips & Extracurricular Activities

I/We authorize my child to participate in school-sanctioned field trips and extracurricular activities organized by St. Bonaventure's College. I/We acknowledge that participation involves inherent risk and agree to hold the College, its staff, and representatives harmless from any injury, loss, or claim arising from such participation.

Terms & Conditions Acknowledgement

I/We have read and agree to St. Bonaventure's College's Admissions Terms & Conditions, Student Code of Conduct, and relevant school policies, copies of which are available at www.stbons.ca or upon request from the Admissions Office. I/We understand that an offer of admission is conditional upon satisfactory review of all submitted documentation and that acceptance of the Hockey Skills Program and Music Program is separate from general admissions.
